Opening a Case

Use this form to report pro bono work for direct representation cases only.
In order to have your pro bono work covered through the Pro Bono Program's malpractice policy, you must submit this form within one week of your representation commencing.

To qualify for coverage, you must:

1. Certify that your client is a person of limited means (at or below 250% of the Federal Poverty Guidelines); and

2. Certify that you are performing legal services at no fee or at a substantially reduced fee.
